Objectifs Les répercussions d’un premier épisode psychotique (PEP) peuvent s’avérer très éprouvantes pour la personne qui en souffre et son entourage. Le processus de rétablissement demeure laborieux et certaines personnes éprouvent de la difficulté à s’y engager. Mieux comprendre les effets de l’intervention de groupe chez les personnes souffrant de symptômes dits négatifs ou encore chez ceux plus réfractaires aux traitements usuels apparaît nécessaire. La thérapie par l’aventure se distingue parmi les interventions groupales par ses activités en plein air, dans un cadre naturel, visant des changements comportementaux. La présente étude vise à mieux comprendre les phénomènes groupaux émergents, ainsi que les fonctions d’un groupe de jeunes adultes psychotiques ayant participé à un programme de thérapie par l’aventure. Le programme comptait 4 semaines de rencontres préparatoires suivies de 4 jours d’expédition en nature et d’une rencontre bilan postexpédition.
Méthode Cette étude qualitative s’appuie sur l’observation participante réalisée tout au long du programme, ainsi que sur des entrevues semi-dirigées menées auprès de 8 participants ayant vécu un PEP, jusqu’à 6 mois après l’expédition. Le matériel de recherche a été analysé à l’aide d’une analyse thématique, puis d’une analyse interprétative de type phénoménologique, afin d’approfondir la compréhension de l’expérience vécue par les participants.
Résultats Les analyses ont mis en lumière 8 thèmes clés, 4 centrés sur les phénomènes groupaux ayant émergé au cours de l’expédition (entraide et initiative en contexte interpersonnel, effet motivationnel, effet déstigmatisant, solidarité) et 4 sur les fonctions de la dynamique groupale (contenance, régulation, médiation, contemplation).
Discussion La thérapie par l’aventure semble favoriser une mise en action et la reprise de confiance en soi chez les participants en plus de les aider à surmonter des difficultés relationnelles importantes en lien avec leur diagnostic de santé mentale. La structure du programme, la présence continue de professionnels en santé mentale, ainsi que la présence de pairs ayant un parcours de vie similaire semblent sécurisantes et permettre de briser l’isolement, de socialiser, de développer des compétences en régulation émotionnelle, d’amorcer des réflexions quant aux habitudes de vie et, pour certains, de changer ces habitudes suite à l’expédition.
Conclusion Cette étude souligne le potentiel thérapeutique de la thérapie par l’aventure, qui peut être bénéfique et complémentaire au traitement usuel dans le traitement global d’un PEP.

Abstract
Ojectives The repercussions of a first episode psychosis (FEP) can be very trying for people suffering from it and those around them. The recovery process remains laborious and some people have difficulty engaging in it. Understanding better the effects of group intervention on people suffering from so-called negative symptoms or on those more resistant to usual treatments appears necessary. Adventure therapy stands out among group interventions for its outdoor activities, in a natural setting, aimed at behavioral changes. This study aims to better understand emerging group phenomena, as well as the functions of a group of young psychotic adults who participated in an adventure therapy program. The program included four weeks of preparatory meetings, followed by a four-day expedition in nature and a postexpedition debriefing session.
Method This qualitative study is based on participant observation conducted throughout the program, as well as semi-structured interviews with 8 participants who experienced a FEP, carried out up to 6 months after the expedition. The research material was analysed using a thematic analysis, followed by an interpretative phenomenological analysis, in order to deepen the understanding of participants’ lived experience.
Results The analyses highlighted 8 key themes, 4 focused on group phenomena that emerged during the expedition (mutual support and initiative in interpersonal context, motivational effect, destigmatizing effect, solidarity) and 4 on the functions of group dynamics (containment, regulation, mediation, contemplation).
Discussion Adventure therapy appears to promote setting in motion and the recovery of self-confidence among participants in addition to helping them overcome significant relational difficulties related to their mental health diagnosis. The structure of the program, the continuous presence of mental health professionals, as well as the presence of peers with a similar life path, seem reassuring and to allow for breaking isolation, socializing, developing emotional regulation skills, initiating reflections on lifestyle habits and, for some, changing these habits following the expedition.
Conclusion This study highlights the therapeutic potential of adventure therapy, which can be beneficial and complementary to usual treatment in a global approach to the treatment of a FEP.
