Résumé

La prise de décision infirmière à l’urgence constitue un processus complexe, particulièrement pour les personnes infirmières novices, en raison de leur expérience clinique limitée. Ce processus repose notamment sur l’évaluation de la personne soignée, l’analyse des options d’intervention et le choix de l’intervention la plus appropriée. Dans un contexte marqué par des ressources humaines et financières limitées, ainsi que par une pénurie de personnel infirmier expérimenté, le soutien offert aux personnes infirmières novices peut être réduit, ce qui accentue les défis liés à la prise de décision clinique. Cet article vise donc à mieux comprendre les enjeux associés au processus décisionnel chez les personnes infirmières novices à l’urgence. Pour ce faire, il présente d’abord les concepts théoriques liés à la prise de décision infirmière et aux facteurs qui l’influencent, puis illustre leur mise en application dans un contexte réel à l’urgence, où des algorithmes décisionnels visant à soutenir la prise de décision clinique ont été élaborés et implantés.

Abstract

Nursing decision-making in the emergency department is a complex process, particularly for novice nurses, due to their limited clinical experience. This process relies on the assessment of the patient, the analysis of intervention options, and the selection of the most appropriate course of action. In a context marked by limited human and financial resources, as well as a shortage of experienced nursing staff, the support available to novice nurses may be reduced, thereby increasing the challenges associated with clinical decision-making. This article therefore aims to better understand the issues associated with the decision-making process among novice nurses in the emergency department. To this end, it first presents the theoretical concepts related to nursing decision-making and the factors that influence it, and concludes with their application in a real emergency department setting, during which decision-making algorithms designed to support clinical decision-making were developed and implemented.
