Abstract
Background: In response to evolving health care demands and the increasing need for accessible patient care, eight Canadian provinces have expanded the scope of practice for registered nurses (RNs) to prescribe medications. Ontario is the latest province to approve RN prescribing. RNs with prescribing authority in Ontario can prescribe specific medications approved by the Ministry of Health and Long-Term Care and authorized by the College of Nurses of Ontario (CNO). RNs must complete one of the four available CNO Council–approved RN prescribing education programs to become an authorized prescriber in Ontario. As RN prescribing is new to Ontario, it is essential to assess the effectiveness of the educational programs preparing nurses for this new role.
Purpose: A quality improvement project (QIP) was conducted to gain learners’ perspectives on one of Ontario’s CNO-approved RN Prescribing programs.
Methods: This QIP focused on learners enrolled through the University Consortium RN Prescribing Program. Pre- and post-course surveys were administered via Qualtrics XM. Data collected included demographic information, motivation for enrolling in the program, employment plans related to the expanded scope of practice, familiarity with the new CNO Practice Standards, and confidence in prescribing safely. Pre- and post-course survey composite scores were computed to reflect participants’ overall familiarity with the RN prescribers’ scope of practice and their confidence in their ability to prescribe safely within the scope of practice. We conducted Mann-Whitney U non-parametric tests were conducted to determine any statistical difference between the median of the pre- and post-course composite scores (familiarity scores and confidence level).
Results: A total of 194 pre-course and 136 post-course anonymous survey questionnaires were completed through Qualtrics XM. The baseline demographic characteristics were similar in pre- and post-course groups. Top motivators for taking the course were career advancement and personal goals. The median familiarity score for pre-course (2.50) and post-course (3.0) surveys was statistically significant (U = 4141.5, z = –10.93, p < 0.001). The median confidence score for pre-course (80.5) and post-course (85.0) surveys was not statistically significant (U = 11629, z = –1.843, p = –0.065).
Conclusion: Upon completion of the course, learners in the University Consortium’s inaugural RN prescribing program reported increased familiarity with the scope of practice and confidence in their ability to prescribe safely. Introducing safe registered RN prescribing in Ontario can significantly advance health care by optimizing resource use, improving access to care, and enhancing patient-centred care.

Contexte : En réponse à l’évolution des demandes en matière de soins de santé et au besoin croissant de soins accessibles aux patients, huit provinces canadiennes ont élargi le champ de pratique des infirmières et infirmiers autorisés (IA) pour leur permettre de prescrire des médicaments. L’Ontario a été la dernière province à autoriser les prescriptions par les IA, qui peuvent dorénavant prescrire des médicaments particuliers approuvés par le ministère de la Santé et des Soins de longue durée et autorisés par l’Ordre des infirmières et infirmiers de l’Ontario (OIIO). Les IA doivent suivre l’un des quatre programmes de formation sur la prescription infirmière approuvés par le Conseil de l’OIIO pour devenir prescriptrices ou prescripteurs autorisés en Ontario. La prescription par les IA étant une pratique nouvelle en Ontario, il est essentiel d’évaluer l’efficacité des programmes de formation qui préparent les infirmières et infirmiers à ce nouveau rôle.
Objectif : Un projet d’amélioration de la qualité (PAQ) a été mené pour recueillir le point de vue des apprenantes et apprenants sur l’un des programmes de formation à la prescription par les IA approuvés par l’OIIO de l’Ontario.
Méthode : Ce PAQ s’est centré sur les apprenantes et apprenants inscrits au University Consortium RN Prescribing Program (Programme de prescription par les IA du Consortium universitaire). Des sondages pré et post-cours ont été effectués au moyen de Qualtrics XM. Les données recueillies comprenaient des renseignements démographiques, la motivation à s’inscrire au programme, les plans d’emploi liés au champ d’exercice élargi, la familiarité avec les nouvelles normes de pratique de l’OIIO et la confiance de pouvoir prescrire en toute sécurité. Les scores combinés des sondages avant et après les cours ont été calculés afin de refléter la familiarité globale des participantes et participants avec le champ d’exercice des IA prescripteurs et leur confiance en leur capacité à prescrire en toute sécurité dans ce champ d’exercice. Nous avons utilisé le test non paramétrique U de Mann–Whitney afin d’identifier toute différence statistique entre la médiane des scores combinés avant et après les cours (scores de familiarité et niveau de confiance).
Résultats : Un total de 194 questionnaires avant et 136 questionnaires après le cours ont été remplis de façon anonyme, par l’entremise de Qualtrics XM. Les caractéristiques démographiques de départ des groupes avant-après étaient similaires. Les principaux facteurs de motivation pour suivre le cours étaient l’avancement dans la carrière et les objectifs personnels. Le score médian de familiarité pour les questionnaires avant les cours (2,50) et après les cours (3,0) était statistiquement significatif (U = 4 141,5; z = –10,93; p < 0,001). Le score médian de confiance pour les questionnaires avant les cours (80,5) et après les cours (85,0) n’était pas statistiquement significatif (U = 11 629; z = –1,843; p = –0,065).
Conclusion : À la fin du cours, les apprenantes et apprenants du premier programme de formation à la prescription infirmière du Consortium universitaire ont déclaré mieux connaître leur champ d’exercice et avoir davantage confiance en leur capacité à prescrire en toute sécurité. L’introduction de la prescription infirmière sécuritaire par les IA en Ontario peut faire progresser considérablement les soins de santé en optimisant l’utilisation des ressources, en améliorant l’accès aux soins et en rehaussant les soins centrés sur le patient.
