Résumé
L’alliance thérapeutique (AT) est considérée comme un des piliers fondateurs d’une démarche thérapeutique. Sa rupture est un phénomène complexe touchant tant les patient(e)s que les thérapeutes. La rupture d’une AT fait intervenir des dynamiques de mise en retrait ou de confrontation, voire les deux. Cet article se focalise sur l’expérience des patient(e)s face à une rupture de l’AT au travers d’entretiens. Que disent les patient(e)s des comportements qu’ils/elles jugent antinomiques au processus thérapeutique ? Comment comprennent-ils/elles la rupture de l’AT – dans un cadre hospitalier ou un suivi privé ? Une résolution est-elle possible ? L’analyse de ces entretiens permet de dresser une cartographie préliminaire de situations types vécues par les patient(e)s et des motifs les ayant conduits à rompre l’AT.
Méthodes Cette étude s’inscrit dans une approche qualitative en sciences sociales. Dans un premier temps, 5 entretiens ont été effectués par une chercheuse experte par expérience en psychiatrie auprès des personnes concernées (toutes de femmes) par une rupture de l’AT. Dans un second temps, les données récoltées ont été transcrites, analysées puis regroupées et comparées par thèmes pour être ensuite réorganisées, fusionnées et enfin hiérarchisées.
Résultats L’analyse a mis en évidence 5 thèmes essentiels dans les facteurs de rupture de l’AT : le pouvoir des professionnels, la relation comme violence symbolique, l’acte de soin, le fonctionnement des institutions de soin et la réparation de l’alliance. Un élément central des discours des patientes interrogées est la violence provoquée par l’expérience vécue d’une asymétrie de pouvoir entre elles et le cadre psychiatrique. Face à cette situation, les patientes mettent progressivement en place des stratégies d’influence pour réduire leur impuissance vécue et adapter le contexte de soin à leurs besoins, comme par la dissimulation d’informations ou le mensonge. Ces récits viennent interroger le bien-fondé des actes de soin en psychiatrie et invitent à remettre en question les déterminants épistémologiques ayant abouti à ces standards et actes de soin en premier lieu.
Discussion En institution psychiatrique, ces résultats suggèrent que les déterminants de l’AT sont en réalité reliés de manière non négligeable aux enjeux de pouvoir à de multiples niveaux. Le maintien, ou la rupture d’une AT s’insère dans un registre sociologique au travers duquel les patientes se reconnaissent en tant que dominées, stigmatisées et marginalisées. Dans ce cadre d’oppression, dissimuler la vérité ou la manipuler apparaît comme des actes nécessaires et licites pour échapper à des situations vécues comme injustes. Ce type de réaction n’est pas nouveau, il s’inscrit dans le développement de l’épistémologie « des dominés », un mouvement en sciences sociales théorisant la construction du savoir chez les personnes et groupes connaissant la domination et où la résistance à l’oppression constitue un acte subversif de lucidité. Il n’est pas étonnant que leurs perspectives soient minoritaires, comme elles reflètent également la position sociale minoritaire des personnes qui la pratiquent.
Conclusion La conservation, ou la réparation d’une AT solide est une priorité pour toute relation thérapeutique. Dans ce cadre, la sensibilisation du personnel psychiatrique aux limites du paradigme biomédical positiviste duquel l’éthos et l’habitus psychiatriques sont profondément imprégnés pourrait être un préalable bienvenu à l’intégration de certains points de vue de l’épistémologique des dominés dans la relation thérapeutique. L’accueil de ces perspectives minoritaires permettrait de nourrir de véritables compréhensions nouvelles dans la dynamique de l’AT et de créer des dynamiques d’AT fondées d’abord sur l’humanité qui nous relie tous et moins sur la catégorie à laquelle on appartient.

Abstract
The therapeutic alliance (TA) is considered one of the founding pillars of a therapeutic approach. Its breakdown is a complex phenomenon affecting both patients and therapists. The breakdown of a TA involves dynamics of withdrawal or confrontation, or both. This article focuses on patients’ experiences of TA breakdown through interviews. What do patients have to say about behaviors that are antinomic to the therapeutic process? How do they understand the breakdown of TA-in a hospital setting or in private care? Is a resolution possible? Analysis of these interviews enables us to draw up a preliminary map of typical situations experienced by patients and the reasons that led them to break off the TA.
Methods This study is part of a qualitative social science approach. In the first stage, 5 interviews were carried out by a researcher with experience in psychiatry with people affected (all women) by a breakdown in TA. The data collected was then transcribed, analyzed, grouped by theme, compared by theme, reorganized, merged and prioritized.
Results The analysis highlighted five key themes in the factors leading to TA breakdown: the power of professionals, the relationship as symbolic violence, the act of care, the functioning of care institutions and the repair of the alliance. A central element in the discourse of the patients interviewed is the violence provoked by the lived experience of an asymmetry of power between them and the psychiatric setting. Faced with this situation, the patients gradually implement strategies of influence to reduce their powerlessness and adapt the care context to their needs, such as withholding information or lying. These narratives call into question the validity of psychiatric acts of care, and invite us to question the epistemological determinants that led to these standards and acts of care in the first place.
Discussion In psychiatric institutions, these results suggest that the determinants of TA are, in fact, not insignificantly linked to power issues at multiple levels. Maintaining or breaking a TA is part of a sociological register in which patients recognize themselves as dominated, stigmatized and marginalized. In this context of oppression, concealing or manipulating the truth appear as necessary and licit acts to escape from situations experienced as unjust and confining. Taking a step back, this type of reaction is not new; it’s part of the development of the epistemology of the dominated, a movement in social science theorizing the construction of knowledge among people and groups experiencing domination, and where resistance to oppression constitutes a subversive act of lucidity. Not surprisingly, their perspectives are in the minority, as they also reflect the minority social position of the people who practice them.
Conclusion The preservation, or repair, of a solid TA is a priority for any therapeutic relationship. In this context, sensitizing psychiatric staff to the limits of the biomedical-positivist paradigm with which the psychiatric ethos and habitus are deeply imbued could be a welcome prelude to integrating certain points of view from the epistemology of the dominated into the therapeutic relationship. Welcoming these minority perspectives (for practitioners, not patients), has the potential to nurture genuinely new understandings in the TA dynamic, and to (re-)create TA dynamics based first and foremost on the humanity that connects us all, and less on the category to which we belong.
