La schizophrénie est un trouble mental sévère, ayant de fortes répercussions fonctionnelles, pouvant être décrit à travers 3 dimensions principales : les symptômes positifs (c.-à-d. hallucinations ou idées délirantes), les symptômes négatifs (p.ex. amotivation, anhédonie) et les symptômes de désorganisation (du discours ou du comportement) (American Psychiatric Association [APA], 2013). Un autre symptôme fréquent, qui freine sérieusement le rétablissement, est souvent occulté dans la recherche et la pratique entourant la schizophrénie : la fatigue. S’il n’existe pas de consensus sur la définition de la fatigue dans la littérature, celle-ci peut être définie comme un épuisement physique, cognitif émotionnel ou un manque d’énergie (Engberg et al., 2017 ; Jason et al., 2010 ; Lukkahatai et Saligan, 2013). Même s’il existe très peu d’écrits abordant les liens entre la fatigue et la schizophrénie, 60 % des personnes ayant ce diagnostic rapportent faire l’expérience d’une fatigue significative (Waters et al., 2013 ; Poole-Wright et al., 2025) interférant dans leurs activités quotidiennes et ayant un impact sur leur santé et leur fonctionnement social (Waters et al., 2013). Cet article d’opinion s’appuie sur une analyse critique de la littérature récente dans le domaine, notamment les revues de Poole-Wright et al. (2025) et Mulin et al. (2023), et sur les connaissances exhaustives du domaine d’un des auteurs (SR). Cette synthèse, destinée aux chercheurs et aux cliniciens, dresse un portrait des causes connues et méconnues de la fatigue, ainsi que des pistes d’intervention, afin de souligner le manque crucial de recherche et d’attention clinique dans ce domaine. Tout d’abord, il est important de distinguer les causes primaires des causes secondaires de la fatigue. Cette distinction permet d’attribuer les causes primaires aux altérations neurobiologiques qui sous-tendent les manifestations psychopathologiques et les processus secondaires aux causes externes qui ont un impact sur la fatigue (Kirschner et al., 2017). Pour les causes primaires, il a été suggéré que la fatigue pourrait être induite par des anomalies cérébrales, une dysbiose intestinale et une réponse immuno-inflammatoire (Mulin et al., 2023). Une autre cause primaire pourrait être la présence de troubles du sommeil chez les personnes avec schizophrénie. Plusieurs études portant notamment sur les différences dans les paramètres du sommeil (phase du sommeil paradoxal [REM] et non-REM) suggèrent que les problèmes de sommeil chez ces derniers reflètent des altérations neurobiologiques intrinsèques au trouble, indépendamment des traitements médicamenteux (Chouinard et al., 2004 ; Guénolé et al., 2014 ; Poulin et al., 2008). Les troubles du sommeil, notamment l’insomnie, toucheraient près de 44 % des personnes avec une schizophrénie (contre 15 % dans la population générale) (Palmese et al., 2011) et entrainent de la fatigue (Waite et al., 2016). Il existerait un lien entre l’organisation du sommeil non paradoxal et les capacités attentionnelles (Forest et al., 2007), indiquant qu’une des causes possibles de la fatigue ressentie par cette clientèle pourrait être d’origine cognitive due à un sommeil altéré. Concernant les facteurs secondaires, la méta-analyse de Poole-Wright et al. (2025) recense seulement 57 articles, dont la majorité (27 articles) porte sur l’association entre fatigue et médicaments antipsychotiques. En effet, si la prise d’antipsychotiques est la pierre angulaire du traitement de la schizophrénie (Société québécoise de la schizophrénie, s.d.), de nombreux patients se plaignent de la sédation/fatigue provoquée par celle-ci (Meehan et al., 2011 ; Read et William, 2019). Toutefois, la taille d’effet rapportée entre fatigue et antipsychotiques est petite, suggérant l’implication d’autres déterminants causaux.
