Résumé
Objectifs La stigmatisation des personnes avec un trouble mental perdure et leur retour au travail (RaT) ou accès à l’emploi reste complexe. Ces stéréotypes négatifs sont souvent internalisés par les personnes concernées au point qu’ils s’autostigmatisent. La littérature montre que les secteurs médical et entrepreneurial entretiennent de nombreux préjugés et appréhensions à leur égard. En revanche, les milieux de la réadaptation et de l’accompagnement vers l’emploi, qui côtoient fréquemment ces personnes et utilisent les approches du rétablissement, manifestent moins de résistances. Bien que la stigmatisation de ces publics soit relativement bien documentée, peu d’études se sont consacrées à différencier les représentations des acteurs responsables de la mise en oeuvre de leur retour et réintégration au travail. Cette étude vise à identifier et comparer les tendances de ces acteurs à stigmatiser les personnes avec un trouble de mental ainsi que leurs intentions d’inclusion professionnelle.
Méthode Dans le cadre de l’enquête internationale de la communauté de pratiques en santé mentale et travail (CoP-SMT), les données ont été collectées par le biais d’une enquête quantitative en ligne auprès de 586 acteurs du retour et de la réinsertion au travail. Quatre systèmes d’acteurs ont été identifiés, ceux de la santé, de l’entreprise, de l’accompagnement vers l’emploi et des personnes avec un trouble mental en absence maladie ou en recherche d’emploi. Le questionnaire comporte une échelle de stigmatisation à 2 facteurs (fardeau organisationnel perçu et émotions négatives) et une mesure d’intention d’inclusion professionnelle à 3 indicateurs (évitement, paternalisme et collaboration). Des analyses factorielles et de cohérence interne ont validé les qualités psychométriques de ces mesures. Des analyses de variances avec comparaisons à posteriori ont permis de comparer les réponses des catégories de 20 acteurs ou plus, soit un échantillon réduit à 473 participants.
Résultats Les résultats mettent en évidence un effet significatif du système d’acteurs sur la stigmatisation et l’intention d’inclusion professionnelle. Ils révèlent que les acteurs qui interviennent en amont de l’emploi (réadaptation et accompagnement vers l’emploi) sont moins enclins à stigmatiser et plus ouverts à inclure les personnes en retour ou en réinsertion au travail que les acteurs qui interviennent dans l’entreprise (hiérarchie, collègues et médecins/infirmier(ère)s du travail). Ils montrent aussi la persistance de l’autostigmatisation des personnes avec un trouble de la santé mentale en retour comme en réinsertion au travail.
Conclusion Pour éclairer les résultats, la question de l’expérience de côtoiement et d’inclusion dans le milieu de travail ordinaire des personnes ayant un trouble mental est abordée à la lumière du rôle des différents acteurs. La connaissance, les orientations conceptuelles autour de la santé mentale et le travail collaboratif en pluridisciplinarité sont suggérés comme leviers antistigmatisation. Des perspectives de recherche et des recommandations sont formulées sur cette base.

Abstract
Objectives The stigma surrounding individuals with mental conditions persists, making their return to work or access to employment complex. These negative stereotypes are often internalized by the affected individuals, leading to self-stigmatization. Literature shows that the medical and entrepreneurial sectors harbor many prejudices and apprehensions towards these individuals. In contrast, the fields of rehabilitation and employment support, which frequently interact with these individuals and use recovery approaches, show less resistance. Although the stigma of these populations is relatively well-documented, few studies have differentiated the perceptions of actors responsible for implementing their return and reintegration into work. This study aims to identify and compare these actors’ tendencies to stigmatize individuals with mental conditions and their intentions of professional inclusion.
Method As part of the international survey by the community of practice in mental health and work (CoP-SMT), data was collected through an online quantitative survey involving 586 actors in the return to work and reintegration processes. Four actor systems were identified: health, corporate, employment support, and individuals with mental disorders on sick leave or job seeking. The questionnaire included a two-factor stigma scale (perceived organizational burden and negative emotions) and a professional inclusion intention measure with three indicators (avoidance, paternalism, and collaboration). Factor analysis and internal consistency validated the psychometric qualities of these measures. Variance analyses with post-hoc comparisons allowed the comparison of responses from categories of 20 or more actors, reducing the sample to 473 participants.
Results The results highlight a significant effect of the actor system on stigma and professional inclusion intention. They reveal that actors involved before employment (rehabilitation and employment support) are less prone to stigmatize and more open to including individuals returning or reintegrating into work than those within the company (hierarchy, colleagues, and occupational health professionals). They also show the persistence of self-stigmatization among individuals with mental health issues returning or reintegrating into work.
Conclusion To better interpret the results, the experience of coexistence and inclusion in the ordinary work environment of individuals with mental health conditions is explored, considering the roles of various stakeholders. Knowledge, conceptual orientations around mental health, and collaborative multidisciplinary work are suggested as anti-stigma strategies. Based on these findings, research perspectives and recommendations are formulated.
