Résumé
Objectifs Le mode relationnel et les difficultés d’engagement des personnes présentant un trouble de personnalité limite (TPL) peuvent susciter diverses émotions chez le clinicien. L’activation émotionnelle et la fatigue de compassion sont ainsi courantes et peuvent conduire à des interventions contre-productives. Le travail auprès de cette clientèle requiert de pouvoir réguler des émotions négatives et inhiber les comportements qui y sont associés. Or, les processus de ce travail émotionnel sont rarement explicités et encore moins enseignés. La formation Projet TANGO a été élaborée pour aider les cliniciens à modifier leurs réactions émotionnelles et comportementales. Des stratégies de la thérapie dialectique comportementale leur sont enseignées afin qu’ils les utilisent eux-mêmes pour se réguler dans le contexte d’intervention émotionnellement exigeante. Cette étude vise à évaluer les effets de cette formation sur les habiletés et la disposition à intervenir.
Méthode La formation Projet TANGO a été évaluée à partir d’un devis mixte auprès de 184 intervenants travaillant auprès d’adolescent et d’adultes en protection de la jeunesse (PJ) et en CLSC. Des mesures de régulation des émotions, des attitudes envers les personnes ayant un TPL, de la qualité de vie professionnelle et de l’estime de soi en tant que travailleur auprès de cette clientèle ont été effectuées avant, après et 3 mois après la formation. Des groupes de discussion pré et postformation ont permis de documenter les défis de la prise en charge de cette clientèle et d’apprécier les effets plus difficilement quantifiables.
Résultats L’ensemble des mesures prétests montrent des scores plus favorables chez les intervenants PJ comparativement aux cliniciens en CLSC. Les analyses qualitatives suggèrent qu’ils utilisent des stratégies de régulation émotionnelle, telles masquer l’émotion véritable et simuler une autre émotion, lesquelles ont été associées à l’épuisement professionnel. Les cliniciens en CLSC reconnaissaient plus aisément les défis émotionnels auprès de cette clientèle. Chez les intervenants PJ, les effets de Projet TANGO sont significatifs pour les variables associées à la disposition à intervenir suggérant des Perceptions et des Attitudes plus positives envers la clientèle (p = 0,011, ηp2 = 0,160 et p = 0,036, ηp2 = 0,120), plus de Satisfaction par la compassion et une diminution de l’Usure de compassion (p = 0,001, ηp2 = 0,222 et p = 0,002, ηp2 = 0,212) 3 mois après la formation. Les cliniciens en CLSC ont obtenu des bénéfices sur leurs Perceptions (p < 0,001, ηp2 = ,168), leurs Attitudes envers le TPL (p < 0,001, ηp2 = 0,185) et leur Satisfaction par la compassion (p = 0,042, ηp2 = 0,065) dès la fin de la formation.
Conclusion Cette étude met en évidence que les 2 groupes profitent différemment de la formation. Les résultats suggèrent qu’elle a permis une lecture plus juste des défis de l’intervention chez les intervenants en PJ. En contrepartie, ces dispositions étant préalablement plus présentes chez les cliniciens en CLSC avant la formation, ils semblent avoir bénéficié de la formation pour augmenter leurs habiletés à intervenir.

Abstract
Objectives The relational mode and engagement difficulties of individuals with borderline personality disorder (BPD) can elicit a variety of emotions in the clinician. Emotional activation and compassion fatigue are thus common and can lead to counterproductive interventions. Working with this clientele requires the ability to regulate negative emotions and inhibit associated behaviors. However, the processes involved in this emotional work are rarely made explicit and even less taught. The Project TANGO training was developed to help clinicians modify their emotional and behavioral reactions in the context of common complex interventions. Strategies from dialectical behavioral therapy are taught so that they can use them to self-regulate during emotionally demanding interventions. The purpose of this study is to evaluate the effects of this training on intervention skills and readiness.
Method The Project TANGO training was evaluated using a mixed-methods design with 184 practitioners working with adolescents and adults in youth protection (YP) and CLSC settings. Measures of emotion regulation, attitudes towards people with BPD, quality of professional life and self-esteem as a worker with this clientele were taken before, after and 3 months after the training. Pre- and post-training focus groups were used to document the challenges of working with this clientele and to assess the more difficult to quantify effects.
Results All of the pre-test measures showed more favourable scores for PJ workers than for CLSC clinicians. Qualitative analyses suggest that they use emotional regulation strategies, such as masking the true emotion and simulating another emotion, which have been associated with burnout. Among PJ workers, the effects of Project TANGO were significant for variables associated with readiness to intervene, suggesting more positive Perceptions and Attitudes towards the clientele (p=0.011, ηp2= 0.160 and p=0.036, ηp2= 0.120), more Compassion Satisfaction and a decrease in Compassion Weariness (p=0.001, ηp2= 0.222 and p=0.002, ηp2= 0.212) three months after the training. CLSC clinicians achieved benefits on their Perceptions (p<0.001, ηp2= 0.168), Attitudes towards BPD (p<0.001, ηp2= 0.185) and Satisfaction with Compassion (p=0.042, ηp2= 0.065) upon completion of the training.
Conclusion This study shows that the two groups benefit differently from the training. The results suggest that the training resulted in a more accurate reading of the challenges of intervention among YP practitioners. On the other hand, since these dispositions were more present among CLSC clinicians before the training, they seem to have benefited from the training to increase their intervention skills.
