Les approches psychanalytiques arrivent en deuxième ligne dans les recommandations actuelles concernant la prise en charge des troubles anxieux. Ce constat est justifié par le manque d’études contrôlées pour ces traitements, ainsi qu’en raison d’une adaptation plus tardive de la psychanalyse aux tendances contemporaines de la psychopathologie et de la psychothérapie centrées sur le symptôme. Un tel exemple d’adaptation est le développement récent d’une psychothérapie psychodynamique focalisée sur le trouble panique (PFPP) (Milrod et al., 1997). Cette psychothérapie paraît efficace (Milrod, 2001). Elle ne l’est probablement pas plus, ni moins, que d’autres approches psychothérapiques, si l’on se réfère aux résultats de la recherche empirique à propos de l’efficacité comparative des psychothérapies. La question posée par le titre de cet article est peut-être moins au coeur du débat que ne l’est celle de la nature des ingrédients actifs du processus psychothérapique.
Abstract
Psychoanalytic treatments have been relegated to a secondary status in the current recommendations concerning the treatment of anxiety disorders. This is due to the lack of controlled studies pertaining to these psychotherapies, as well as to the late adaptation of psychoanalysis to the contemporary trends of psychopathology and psychotherapy focusing on symptoms. The recent development of a manualized psychotherapy for the treatment of panic disorder, the Panic-Focused Psychodynamic Psychotherapy (PFPP), is a good example of such an adaptation (Milrod et al., 1997). PFPP appears to be effective (Milrod, 2001). This type of psychotherapy is probably neither more nor less effective than other forms of psychotherapy if one considers the results of empirical research on the comparative effectiveness of psychotherapies. The question formulated by the title of this paper is perhaps less central as is that of the nature of the active factors of the psychotherapeutic process.

Los enfoques psicoanalíticos se encuentran en segundo plano en las recomendaciones actuales relacionadas con hacerse cargo de los problemas de ansiedad. Esta constatación se justifica por la falta de estudios controlados de estos tratamientos, así como por una adaptación más tardía del psicoanálisis a las tendencias contemporáneas de la psicopatología y de la psicoterapia centradas en los síntomas. Un ejemplo de adaptación es el reciente desarrollo de una psicoterapia psicodinámica centrada en el trastorno de pánico (PFPP) (Milrod et al., 1997). Esta psicoterapia parece eficaz (Milrod, 2001). No lo es probablemente ni más ni menos que otros enfoques psicoterapéuticos, si se hace referencia a los resultados de la investigación empírica a propósito de la eficacidad comparativa de las psicoterapias. La pregunta planteada por el título de este artículo es tal vez menos el tema principal del debate que aquella sobre la naturaleza de los ingredientes activos del proceso psicoterapéutico.

As abordagens psicanalíticas chegam em segundo lugar nas recomendações atuais com respeito ao tratamento dos transtornos de ansiedade. Esta constatação é justificada pela falta de estudos comprovados para estes tratamentos, e também por causa da adaptação mais tardia da psicanálise às tendências contemporâneas da psicopatologia e da psicoterapia centradas nos sintomas. Um exemplo desta adaptação é o desenvolvimento recente de uma psicoterapia psicodinâmica focalizada no transtorno do pânico (PFPP) (Milrod e outros, 1997). Esta psicoterapia demonstra ser eficaz (Milrod, 2001). Ela não é provavelmente nem mais nem menos que outras abordagens psicoterápicas, se observarmos os resultados da pesquisa empírica sobre a eficácia comparativa das psicoterapias. A questão levantada pelo título deste artigo é talvez menos centrada no debate que a natureza dos ingredientes ativos do processo psicoterapêutico.
