RÉSUMÉ

Après le tournant pris par la psychiatrie en 1960 et les lenteurs de la mise en place de la politique de secteur, ces dernières années se sont caractérisées par une accélération de la production de textes visant à organiser de manière cohérente la politique de santé mentale en France. En même temps, la réflexion s'est élargie. La question de la santé mentale n'est plus seulement traitée sous l'angle de la structuration modernisée du champ de la psychiatrie, comme si elle pouvait rester dans ses limites traditionnelles. Deux orientations stratégiques se dessinent pour faire sortir les patients et les réponses thérapeutiques d'un espace institutionnel replié jusquelà sur lui-même: d'une part utiliser les dispositifs qui relèvent de l'aide sociale, d'autre part intégrer la psychiatrie à l'hôpital général. Face à cette bipolarisation, comment penser la place spécifique des institutions psychiatriques?

ABSTRACT

Following psychiatry's turning point in 1960, as well as very slow progress in the establishment of a sectorial policy, these last few years have been characterized by the increased production of reports that aim to organize France's mental health policy in a coherent fashion. Meanwhile, thought on the issue has taken on a broader scope. The mental health issue is no longer limited to the structural modernization of the field of psychiatry, in the sense of remaining within traditional boundaries. From this approach arise two strategic orientations designed to let go patients and arise two strategic orientations designed to let go patients and find therapeutic responses from an institutional environment that has long kept to itself. The first orientation deals with mechanisms that are based in social support interventions, while the second focuses on integrating psychiatry in general hospitals. Faced with this bipolarization, how will psychiatric institutions fit in the overall scheme of things?
