Résumé

Dans cet article, les auteurs analysent l'évolution et le développement du concept de communauté thérapeutique dans le monde anglo-saxon. Dans une première partie, ils définissent ce concept et ses caractéristiques. Ensuite, ils explicitent les concepts-clés sous-jacents à l'intervention thérapeutique dans une communauté typique (l'hôpital Belmont de Maxwell Jones), soit l'organisation sociale, le leadership, la prise de décision, le feedback et l'apprentissage social. Dans une deuxième partie, ils explicitent les critiques de Mosher et Menn, de Berke et de Sassolas envers le traitement de la psychose pratiqué dans le système de soins institutionnel, critiques qui peuvent se résumer sous les termes suivants : le modèle théorique prédominant (le modèle médical), le nombre, la structure sociale et l'utilisation des médicaments. Finalement, ils illustrent l'approche proposée par ces thérapeutes-théoriciens, approche basée sur la conception que la psychose est compréhensible.

SUMMARY

In this article, the authors analyze the evolution and development of the therapeutic community concept in the Anglo-Saxon world. In an initial section, they define this concept and its characteristics. Then they make explicit the key concepts which underlie therapeutic intervention in a typical therapeutic community : that of Maxwell Jones at Belmont Hospital. Finally, on the basis of the work of Mosher and Menn, of Berke, and of Sassolas, they elucidate the criticisms made by the above mentionned authors with regard to the treatment of psychosis practised in the institutional treatment system, and they illustrate the approach proposed by these therapist-theoreticians.
