Déjà, en 1861, Windell Holmes faisait remarquer que la médecine est aussi sensible aux influences politiques, religieuses, philosophiques que le baromètre l'est à la pression atmosphérique. La médecine, ou mieux son exercice, de même que la santé sont aujourd'hui des sujets importants du discours politique tant pour l'établissement de programmes de soins, préventifs ou curatifs, que pour les décisions budgétaires qui les accompagnent. Dans ce processus, la base scientifique qui sous-tend de telles décisions n'a pas toujours la rigueur qu'elle se devrait d'avoir. En conséquence, des programmes de santé et de bien-être sont établis sans qu'aient été mesurées les conséquences iatrogéniques qu'ils peuvent causer. Le processus politico-social est tel que, lorsqu'un programme a acquis un certain momentum et a obtenu une bénédiction politique, il est quasi impossible d'en changer le cours même si des évidences démontrent que son coût, ou mieux ses effets malencontreux, dépassent de beaucoup ses éléments bénéfiques. Quand une politique soulève des questions idéologiques, elle a tendance à passer rapidement à un stage de validation sociale et d'acquérir sa vie propre, divorcée de tout examen empirique ou de réfutation scientifique. C'est à la lumière de ces quelques considérations que je voudrait poser le problème du malade psychiatrique en 1977.

SUMMARY

The author poses the problem of the chronic mental patient in 1977 in light of the commu-nity approach. During the 1950's the discovery of penicillin, the phenothiazines and the anti-depressants; the notion of institutional dementia as well as the rapid growth of physical health costs, formed the basis for a needed reform which took the guise of community psychiatry. The latter created many hopes (eg. the possible elimination of the asylum, the potential for preventive effects). In fact community mental health programs did produce a decrease in the hospitalized psychiatric population, a higher quality of hospital services, a change in the laws relating to the mentally ill, and an improvement in treatment, personnel competence as well as a stability in real health costs. But the 1970's brought disillusionment. The rate of psychotic disorder remains unchanged, the cost-benefit of the program is less than had been anticipated and, in addition, the community approach has transplanted the asylum to the city thus creating a new immigrant — the psychotic individual. Citing this failure, the author argues for a psychiatry focussed on research into the makeup of the internal psychotic dynamic and its treatment.
