La pandémie de COVID-19 a mis à rude épreuve les systèmes de santé de nombreux pays qui ont dû, dans un court laps de temps et dans un climat d’urgence, réorganiser leur offre de services pour répondre aux impératifs de la crise. Au Canada, c’est la province de Québec qui, au vu du nombre d’infections et de décès liés à la COVID-19, a été la plus durement touchée lors de la première vague (Dubois, Borgès da Silva et Prud’homme, 2022), avec un total de 59 845 cas confirmés entre mars et juillet 2020, 7 310 hospitalisations et 5 829 décès (Carazo et al., 2022; CSBE, 2022). Le secteur des centres d’hébergement et de soins de longue durée (CHSLD) a été particulièrement touché, avec plus de 64 % des décès liés à la COVID-19. Certains segments de la population ont été également surreprésentés parmi les cas confirmés, les personnes hospitalisées ou décédées, notamment les personnes moyennement défavorisées et défavorisées selon l’indice de vulnérabilité sociale et de vulnérabilité matérielle (Tremblay et Benigeri, 2020). Les travailleurs de la santé ont également été fortement touchés par la pandémie, avec d’importantes disparités selon la profession. Une enquête menée auprès de 5 000 travailleurs de la santé infectés lors de la première vague révèle que « les médecins représentaient 3 % de ces cas confirmés, tandis que les préposés aux bénéficiaires, les infirmières auxiliaires et les infirmières représentaient 70 % des cas » (Tremblay et Benigeri, 2020, p. 32). Au-delà des données épidémiologiques sur les infections à la COVID-19, les effets de la crise se sont également manifestés à travers l’expérience subjective des professionnels de la santé et des services sociaux, qui ont rapporté une augmentation marquée du stress, de l’anxiété et de l’épuisement professionnel (Pelletier et al., 2021; Dubois, Borgès da Silva et Prud’homme, 2022; Laberge et Djedou, 2024). Ces phénomènes s’expliquent notamment par la surcharge de travail, le manque de personnel et la peur de contracter ou de transmettre le virus. Plusieurs études ont signalé une hausse des congés de maladie pour des raisons psychologiques, en particulier chez les infirmières (Lapointe, 2022; Borgès da Silva, Dubois et Prud’homme, 2023). On observe également un allongement des heures de travail et un recours accru au temps supplémentaire obligatoire (TSO). Le manque de personnel a entraîné le redéploiement de travailleurs vers des secteurs critiques (par exemple : soins intensifs, CHSLD en crise), provoquant une déstabilisation importante des équipes. Par ailleurs, au début de la pandémie, des pénuries d’équipements de protection individuelle (ÉPI) ont été constatées, accentuant le sentiment d’insécurité parmi le personnel (Dubois, Borgès da Silva et Prud’homme, 2022). Ce portrait sommaire apparaît comme la conséquence de failles et de vulnérabilités déjà présentes dans le système de santé et de services sociaux avant la pandémie (Naylor et al., 2015; Forest et Martin, 2018) et qui ont été exacerbées par la crise sanitaire (CSBE, 2022; Denis et al., 2021). L’une des vulnérabilités tient au fait que le système de santé et des services sociaux a, comme plusieurs autres juridictions, historiquement privilégié un modèle hospitalo-centré, caractérisé par une concentration des ressources dans les établissements de soins aigus (Hébert, Sully et Nguyen, 2017; Usher et al., 2021) au détriment des soins de première ligne (PL), incluant le secteur des CHSLD.
