Résumé
Les théories métaboliques de divers troubles psychiatriques suscitent un intérêt croissant dans la communauté médicale. Celles-ci sont soutenues par une littérature scientifique émergente suggérant un rôle central des dysfonctionnements énergétiques neuronaux dans diverses psychopathologies. Parallèlement, l’utilisation d’interventions métaboliques gagne en crédibilité dans la prise en charge de troubles psychiatriques, tels que le trouble bipolaire. Nous présentons ici le cas d’une patiente de 34 ans connue pour un trouble bipolaire de type II, un trouble de personnalité mixte ainsi qu’un trouble de l’usage de l’alcool et du cannabis, qui présente une euthymie avec des symptômes résiduels anxio-dépressifs persistants et une altération du fonctionnement global. Dans le cadre d’une approche thérapeutique complémentaire à la psychopharmacologie, la patiente a entrepris une intervention nutritionnelle cétogène d’une durée de 3 mois. À la fin de l’intervention, une amélioration cliniquement significative a été observée, tant sur le plan des paramètres anthropométriques que des symptômes psychiatriques. La patiente a également rapporté une amélioration notable de son niveau de fonctionnement quotidien et de sa qualité de vie. Ce cas contribue à la littérature grandissante soutenant le potentiel des interventions métaboliques en psychiatrie et souligne la pertinence de poursuivre l’exploration systématique de ces approches dans le traitement des troubles de l’humeur.

Abstract
Metabolic theories of various psychiatric disorders are generating growing interest within the medical community. These theories are supported by an emerging scientific literature suggesting a central role of neuronal energy dysfunction in diverse psychopathologies. In parallel, the use of metabolic interventions is gaining credibility in the management of psychiatric disorders such as bipolar disorder. We present here the case of a 34-year-old woman with a known diagnosis of bipolar disorder type II, mixed personality disorder, and alcohol and cannabis use disorder, who exhibits euthymia with persistent residual anxio-depressive symptoms and impaired overall functioning. As part of a therapeutic approach complementary to psychopharmacology, the patient undertook a ketogenic nutritional intervention lasting 3 months. At the end of the intervention, a clinically significant improvement was observed, both in anthropometric parameters and in psychiatric symptoms. The patient also reported a marked improvement in her level of daily functioning and quality of life. This case contributes to the growing body of literature supporting the potential of metabolic interventions in psychiatry and highlights the relevance of pursuing systematic exploration of these approaches in the treatment of mood disorders.
