Résumé
Le trouble de la personnalité narcissique (TPN) et le trouble de la personnalité mixte (limite et narcissique) sont de sérieux problèmes de santé mentale chez les hommes. Ces pathologies sont notamment caractérisées par des réactions intenses à une menace perçue de l’estime de soi et une capacité compromise à être empathique, ce qui prédispose à des difficultés relationnelles importantes. Considérant aussi ces troubles de la personnalité comme étant des troubles de la régulation affective, l’auteur illustre comment la dysrégulation affective stimulée par l’émergence de la honte engendre une perte de la capacité de mentaliser. Cet article présente la mise en place de même qu’une description d’un traitement individuel et de groupe basé sur la mentalisation pour hommes narcissiques. Différentes réflexions sur le traitement, sur l’empathie, ainsi que l’adaptation de cette approche pour des patients vivant avec un TPN sont faites et sont illustrées par des vignettes cliniques.

Abstract
Narcissistic personality disorder (NPD) and not otherwise specified personality disorder (borderline and narcissistic) are major mental health problems for men. These pathologies predispose patients to significant relational difficulties which are characterised by intense reactions to perceived threats to the self-esteem and an impaired empathic ability. Understanding these disorders as affect dysregulation stimulated by shame, the author presents how patients in this context are prone to lose their capacity to mentalize. This article describes a specific individual and group mentalization-based treatment and its implementation for treating narcissistic men. Different reflections on the treatment, on empathy, as well as on the adaptation of this treatment for NPD patients are presented and illustrated with clinical vignettes.
