Résumé
Les auteurs présentent un aperçu de l’éventail des interprétations psychanalytiques de la pathologie narcissique, en mettant l’accent sur le modèle et le système de classification décrits par Kernberg (1984). Ils abordent la manière dont le clivage des dyades relationnelles objectales favorise une importante pathologie, tant du développement que du fonctionnement du surmoi et de l’établissement défensif d’un faux soi grandiose. Ces trois points sont au coeur des pathologies du narcissisme. Les auteurs passent en revue les modifications psychothérapiques susceptibles d’aider les cliniciens à soigner efficacement les patients souffrant d’une pathologie du narcissisme. Ils présentent les distinctions entre les divers niveaux de narcissisme qui influencent à la fois l’approche thérapeutique et le pronostic.
Abstract
In this paper, the authors present an overview of a range of psychoanalytic understandings of narcissistic pathology with an emphasis on the model and system of classification described by Kernberg. They discuss how the concept of a fundamental split between object relations dyads can contribute to major pathology of superego development and functioning and to the defensive establishment of the false grandiose self that are central to pathological narcissism. The authors also review therapeutic modifications that can help clinicians effectively treat patients with narcissistic pathology and describe distinctions in levels of narcissism that influence both how to approach treatment and prognosis.
Resumen
Los autores presentan un resumen de la gama de interpretaciones psicoanalíticas de la patología narcisista, enfatizando el modelo y el sistema de clasificación descritos por Kernberg (1984). Abordan la manera en que la división de las díadas objetales relacionales favorece a una importante patología, tanto del desarrollo como del funcionamiento del superego y del establecimiento defensivo de un falso ello grandioso. Estos tres puntos se encuentran en el centro de las patologías del narcisismo. Los autores revisan las modificaciones psicoterapéuticas capaces de ayudar a los clínicos a sanar eficazmente a los pacientes que sufren de una patología del narcisismo. Presentan las distinciones entre los diversos niveles de narcisismo que influyen a la vez en el enfoque terapéutico y el pronóstico.

Os autores apresentam um resumo das várias interpretações psicanalíticas da patologia narcísica, ressaltando o modelo e o sistema de classificação descritos por Kernberg (1984). Eles abordam a maneira pela qual a clivagem das bivalentes relações objetais favorece uma patologia importante, tanto do desenvolvimento quanto do funcionamento do superego e do estabelecimento defensivo de um falso eu grandioso. Estes três pontos estão no centro das patologias do narcisismo. Os autores passam em revista as mudanças psicoterápicas suscetíveis de ajudar os profissionais clínicos a tratar eficazmente os pacientes que sofrem de uma patologia do narcisismo. Eles apresentam as distinções entre os diversos níveis de narcisismo que influenciam, ao mesmo tempo, a abordagem terapêutica e o prognóstico.
