Résumé
Les patients aux prises avec un trouble de personnalité limite ont par définition une personnalité non fonctionnelle sujette à réagir aux changements et aux stress psychosociaux par des états de crise récurrents. Un trouble de personnalité est indissociable d’une problématique interpersonnelle et celle-ci se développe forcément dans le cadre d’une psychothérapie psychodynamique individuelle. Cet article aborde les principes de base du cadre psychothérapique permettant de structurer l’intervention de crise adaptée au trouble de personnalité limite lors des incidents rencontrés au cours du traitement. Ce cadre sert de mesure préventive et/ou structurelle face aux crises et offre ainsi des stratégies générales de résolution. L’article détaille des problématiques cliniques fréquentes soit des menaces de suicide et même d’homicide, des conflits provoqués par les demandes du patient, l’absence du thérapeute ou des tentatives visant à perturber le cadre afin de décrire diverses modalités d’intervention pour solutionner ces crises.
Abstract
Patients suffering from borderline personality disorder have by definition a poorly functional personality which is readily reacting to changes and psychosocial stressors with recurrent crises. A personality disorder is necessarily fraught with interpersonal difficulties, and it is often within the individual psychotherapeutic encounter that such clashes develop. This article discusses basic principles offering clinicians ways to prevent or structure crisis intervention adapted to borderline personality disorder within the individual psychotherapy context. It thus reviews various clinical issues such as suicidal or homicidal threats, conflicts related to requests expressed by the patient, the absence of the therapist or attempts at breaking the therapeutic framework in order to offer intervention modalities for the resolution of these crises.
Resumen
Los pacientes que sufren de un trastorno de personalidad límite tienen por definición una personalidad disfuncional susceptible de reaccionar a los cambios y al estrés psicosocial por los estados de crisis recurrentes. Un trastorno de personalidad es indisociable de una problemática interpersonal y ésta se desarrolla forzosamente en el marco de una psicoterapia de psicodinámica individual. Este artículo aborda los principios de base del marco psicoterapéutico que permite estructurar la intervención de crisis adaptada al trastorno de personalidad límite en el momento de los incidentes encontrados en el curso del tratamiento. Este marco sirve de medida preventiva y/o estructural frente a las crisis y ofrece de esta manera las estrategias generales de resolución. El artículo detalla las problemáticas clínicas frecuentes, ya sean amenazas de suicidio e incluso de homicidio, conflictos provocados por las demandas del paciente, la ausencia del terapeuta o las tentativas que buscan perturbar el marco a fin de describir diversas modalidades de intervención para solucionar estas crisis.
Resumo
Os pacientes que sofrem de um Transtorno de Personalidade Limítrofe têm, por definição, uma personalidade não-funcional sujeita a reagir às mudanças e ao estresse psicossociais através de estados de crise recorrentes. Um transtorno de personalidade é indissociável de uma problemática interpessoal e esta desenvolve-se obrigatoriamente dentro de uma psicoterapia psicodinâmica individual. Este artigo aborda os princípios de base do quadro psicoterapêutico permitindo estruturar a intervenção de crise adaptada ao transtorno de personalidade limítrofe durante incidentes encontrados durante o tratamento. Este quadro serve de medida preventiva e estrutural face às crises e oferece, assim, estratégias gerais de resolução. O artigo detalha problemáticas clínicas freqüentes, por exemplo ameaças de suicídio e mesmo de homicídio, conflitos provocados pelos pedidos do paciente, ausência do terapeuta ou tentativas que visam perturbar o quadro para descrever várias modalidades de intervenção para solucionar estas crises.
