La santé mentale au travail : pour une compréhension de cet enjeu de santé publique Mental health at work: for an understanding of this stake in public health

RÉSUMÉ

Dans les pays occidentaux, les problèmes de santé mentale sont en croissance et représentent l'une des principales causes de morbidité de la population, avec une prévalence annuelle qui varie de 15 à 25 %. Les conséquences de cette morbidité se font sentir de façon importante au chapitre de la capacité de travail des individus. Plusieurs éléments liés notamment à l'évolution de l'organisation et des conditions de travail au cours des dernières années incriminent le milieu de travail dans l'explication de l'origine de cette nouvelle « épidémie ». S'il y a une relative unanimité sur l'importance des problèmes de santé mentale au travail, il n'en va pas de même de la compréhension de l'origine de ces problèmes, et par voie de conséquence, des stratégies à mettre en oeuvre pour les contrer. De l'ensemble des recherches qui ont tenté d'expliquer ce phénomène, trois approches sont considérées de façon particulière : l'approche causaliste, l'approche cognitiviste et enfin, l'approche de la psychodynamique du travail. Même si l'approche cognitive permet de comprendre pourquoi certains facteurs de stress identifiés par l'approche eausaliste peuvent être pathogènes, elle apparaît un peu réductionniste en ramenant les problêmes de santé mentale au travail à l'échec des efforts d'adaptation des individus. Contrairement à l'interprétation cognitive et aux actions individuelles auxquelles nous conduit l'approche du stress, la psychodynamique du travail débouche sur un questionnement de l'intelligibilité de l'origine organisationnelle des problèmes de santé mentale au travail, en analysant l'interface dynamique et évolutive entre les objectifs que poursuivent l'individu, l'organisation et le groupe de travail.

ABSTRACT

In western countries, mental health problems are increasing and represent one of the major causes of morbidity of the population with an annual prevalence varying from 15 to 25%. Consequences of this morbidity are more importantly felt on the working capacity of individuals. Several elements linked particularly to the evolution of the organization and working conditions in the course of the last years incriminate the workplace in explaining the origin of this new "epidemic". If there is a relative unanimity on the importance of mental health problems at work, it is not the case with the understanding of the origin of these problems and, consequently, of the strategies to put in place to counter them. Of the entire studies that have attempted to explain this phenomenon, three approaches are considered: the causalist approach, the cognitivist approach and finally, the psychodynamic approach. Even if the cognitivist approach allows to understand why some stress factors identified by the causalist approach can be pathogenic, it appears a bit reductionist by linking mental health problems at work with the failure of people's efforts of adaptation. Contrarily to the cognitivist interpretation and to individual actions which brings us to the approach to stress, the psychodynamic of work leads to a questioning of the intelligibility of the organizational origin of mental health problems at work, by analyzing the dynamic and evolutive interface between the objectives pursued by the individual, the organization and the working group.
